Conducts a thorough review of all abstraction and search of records and guidelines in order to select the most accurate and descriptive codes in accordance with CPT/HCPCS coding system. Uses coding conventions, techniques, coding rules and CMS regulations, keeps up-to-date with current codes and trends. Works with automated record systems, data entry, and encoder software. Quantitative analysis " Performs a comprehensive review of the record to assure the presence of all component parts such as: patient and record identification, signatures and dates where required, and the presence of all reports which appear to be indicated by the nature of the treatment rendered. Qualitative analysis " Evaluates the record for documentation consistency and adequacy. Medico-Legal requirements " Makes final determination that the record is complete, accurate, and reflects sufficient data to justify the diagnosis and warrant treatment and end results, without infringing on decisions concerning a physician's clinical judgment. Responsible for data entry of all clinical record information into the RPMS/Electronic Medical Record system for transmission to the Albuquerque Area Office. Corrects error listings and reports pertaining to coding. Maintains complete confidentiality of all administrative, medical and personnel records and all other pertinent information that comes to his/her attention or knowledge in accordance with the Privacy Act of 1974, Alcohol and Drug Abuse Patient Records, Freedom of Information Act, Health Insurance Portability and Accountability Act (HIPAA) and other pertinent federal regulations. GS-06: In addition to above: Abstracts all necessary information from medical records to identify secondary complications and co-morbid conditions. Provides technical assistance with processing and maintaining CPT coding, abstraction of the complete chart (inpatient/outpatient) and compliance enforcement of all regulatory requirements. Processes into the RPMS PCC System from PCC Ambulatory Encounter forms. Data entry mnemonics are utilized to facility entry of data items such as: Diagnoses, procedures, treatments, patient education, measurements, medications, exams, laboratory/radiology tests ordered/results, immunizations, skin test readings, problems list updates, historical data items and level of service. Orients and trains new employees, practitioners and other hospital staff to their medical record functions according to the Medical Records Manual, Chapter 3 to ensure accuracy of documentation, as needed. GS-07: In addition to the above: Provides education and updates to the medical staff, nursing staff and other health care providers on changing coding conventions, rules, regulations and guidelines. Identifies inconsistencies or discrepancies within the medical record and discusses with the appropriate medical, nursing, or health care providers for corrective action, without infringing on decisions concerning clinical judgment. Performs other related duties as assigned. Qualifications: You must have one year of specialized experience at a level close to the work of this job that has given you the particular knowledge, skills, and abilities required to successfully perform. Typically we would find this experience in work within this field or a field that is closely related. Candidates for GS-5 must have had 52 weeks of Specialized Experience equivalent to GS-4 OR four (4) years above high school. Candidates for the GS-6 must have had 52 weeks of Specialized Experience equivalent to at least the GS-5 grade level. Candidates for the GS-7 must have had 52 weeks of Specialized Experience equivalent to at least the GS-6 grade level. Specialized Experience: Demonstrated experience in a hospital or clinic setting where the candidate has gained knowledge of ICD-9-CM Coding, Health Care Procedural Coding System (HCPCS), Current Procedural Terminology (CPT); and evaluating and managing coding theory guidelines.
