Position Summary

The Patient Care Coordinator at MIORA is a vital member of our healthcare team, responsible for ensuring a seamless and positive experience for patients. This role involves answering patient questions, ordering medications, and educating patients on supplements and medications. The Patient Care Coordinator will serve as a primary liaison between patients and healthcare providers, helping to facilitate comprehensive and coordinated care.

Job Duties/Responsibilities

Patient Interaction and Support:

  • Answer patient questions and provide information about the clinic's services, procedures, and policies.
  • Assist patients with scheduling appointments and follow-up visits.
  • Address patient concerns and provide support to ensure a positive experience

Medication Management:

  • Coordinate and order medications for patients as prescribed by healthcare providers.
  • Ensure timely and accurate processing of medication orders and refills.
  • Communicate with pharmacies to get accurate pricing and resolve any issues related to medication availability.

Patient Education:

  • Educate patients on prescribed medications, including usage instructions, potential side effects, and precautions.
  • Provide information and guidance on recommended supplements, including benefits and proper usage.
  • Develop and distribute educational materials to enhance patient understanding of their treatment plans.

Administrative Duties:

  • Maintain accurate and up-to-date patient records in the clinics electronic health record (EHR) system.
  • Assist with the coordination of patient care, including referrals to specialists and coordination of diagnostic tests.
  • Support the clinics administrative functions, including answering phone calls, managing correspondence, and assisting with front desk operations as needed.

Coordination of Care:

  • Collaborate with healthcare providers, nurses, and other clinic staff to ensure a holistic and integrated approach to patient care.
  • Participate in team meetings and case conferences to discuss patient progress and care plans.
  • Facilitate communication between patients and healthcare providers to ensure clarity and continuity of care.

Minimum Required Qualifications

  • Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare providers.
  • Strong organizational skills and attention to detail, with the ability to manage multiple tasks and priorities.
  • Proficiency in using electronic health record (EHR) systems and other healthcare software.
  • Ability to maintain confidentiality and handle sensitive information with professionalism and discretion.
  • Empathy and compassion for patients, with a commitment to providing high-quality patient care.

Education:

  • Registered Nurse (RN)

Years of Experience:

  • Previous experience in a healthcare setting, particularly in patient care coordination or a similar role, is highly desirable.
  • Knowledge of medical terminology and understanding of integrated healthcare principles.
