Patient Care Coordinator - Care Management Per Diem Day

Patient Care Coordinator - Care Management unit at Providence St. Joseph Hospital in Orange, CA. Positions available are Per Diem and will work 8-hour Day shifts.

Under the direction of the Supervisor or Manager, this position is responsible for providing post discharge appointments for at risk patients discharging from the hospital. This position works closely as a healthcare team member to assist our hospital in providing excellent care and communication to our patients.

Required Qualifications:

• Completion of Medical Terminology coursework or working knowledge of Medical Terminology
• 1 year Healthcare experience

Preferred Qualifications:

• Associate's Degree in related field
• Medical Assistant Certification (CMA) upon hire
• Managed care or case management experience
• Experience with Mcg Care Guidelines, Allscripts, Touchwork, MEDITECH, Enterprise and/or IDX
