Medical Records Clerk (Indiana Ambulatory Surgical Associates) - Full Time

Experience and Education - High School graduate or GED equivalent. One year of clerical experience. Medical Record's experience and/or training required. Basic computer (keyboard) skills. Basic medical/surgical, anatomy and physiology, pathology and medical office procedures. Must possess strong interpersonal skills and be able to communicate effectively with coworkers and be able to work effectively as a team member within the clinic.

Knowledge and Ability - This position requires frequent standing or sitting for prolonged periods of time. Requires eye-hand coordination and manual dexterity necessary for the operation of basic office equipment such as computer terminal and telephone. Requires the ability to work quickly under high stress with patients, fellow employees and physicians. Interaction with others is constant and interruptive.

This person must be able to concentrate for long periods of time, pay close attention to detail, and be very accurate. This person must also be able to communicate and relate well with fellow employees and physicians.

Duties and Functions:

  • Analyze medical records for completeness and accuracy, using the Medical Record Policy for documentation and signature entries.
  • Scan miscellaneous documents into the electronic medical record and notify physicians for review and signature.
  • Monitors the quality and timeliness of medical record documentation and shares findings with the administration.
  • Follows Completion of Medical Record policy to ensure timely completion of medical records.
  • Retrieve and distribute mail to hospital mail room.
  • Disclose health information for treatment and payment consistent with surgery center policies and procedures.
  • Answers telephone, screen visitors and greet patients in a prompt and helpful manner.
  • Scans documents (Medical Record Request) into electronic format for storage.
  • Retrieves copies of records when requested by either patient, health insurance companies, federal agencies or an attorney per company policy.
  • Attends in-services and committee functions relative to medical record keeping functions.
  • Follows HIPAA Privacy and Security Regulations by maintaining confidentiality of patient information.
  • Performs related responsibilities and duties as assigned by the administration.
  • Monitors and assists with monthly physician peer review.
  • Participates in Quality Improvement and assists staff with collection of data.
  • Assist with scheduling, registration and insurance processes.
  • Process and post patient payments into appropriate account.
  • Demonstrates professionalism in appearance and actions.
  • Assumes responsibility for own actions and behaviors.
  • Communicate effectively with coworkers and be able to work effectively as a team member.
