Job Type: Full-time (Monday-Friday, 8:00 AM - 5:00 PM)

POSITION SUMMARY

The Specialty Patient Care Coordinator (SPCC) will be responsible for ensuring that patients receive comprehensive, coordinated care throughout their healthcare journey. The SPCC will work closely with patients, their families, healthcare providers, and other stakeholders to facilitate communication, manage care plans, and provide necessary resources. The ideal candidate will have a background din healthcare and a combination of skills in customer service, referrals, communication, behavioral health, coordinated care and insurance.

Essential Functions Statement(s)

  • Patient Support and Coordination

Develop and manage personalized care plans for patients, addressing their unique needs and preferences.

  • Serve as the primary point of contact for patients and their families, providing guidance and support throughout their care journey.
  • Facilitate communication between patients, healthcare providers, and other stakeholders to ensure timely and accurate information sharing.
  • Care Plan Management
  • Monitor patient progress and adherence to care plans, making adjustments as necessary to ensure optimal outcomes.
  • Coordinate referrals to specialist, behavioral health services, and community resources.
  • Assists patients with scheduling appointments, transportation, and other logistical needs.
  • Identify and address care coordination errors and gaps, working to continuously improve processes and patient outcomes.
  • Training and Development
  • Participate in ongoing training in customer service, referrals, quality, communication/transportation, behavioral health, coordinated care, and insurance.
  • Stay current with best practices and advancements in patient care coordination.
  • Resource Management
  • Connect patients with appropriate insurance, financial assistance programs, and community resources.
  • Provide educational materials and resources to patients and their families, empowering them to make informed decisions about their care.
  • Data Management
  • Maintain accurate and up-to-date patient records and documentation.
  • Other duties as assigned.

SKILLS & ABILITIES

Education: High School Diploma or General Education Degree (GED)

Experience: Experience in customer service, healthcare, or related field required. \
